Accident Reporting Form (for Workers' Compensation): This form is used to report accidents that occur on campus property.
Alternate-Reduced Work Schedule Arrangement: This form is used by employees and supervisors to create an agreement and to describe a work schedule that will vary from the employee's typical work week. This could be on a long-term or short-term basis.
Handbook Acknowledgement and Agreement
These forms are used to show that employees were informed of where to find information regarding Skidmore's policies and procedures.
Department of Homeland Security I-9 Form:This form is used to verify eligibility to work in the United States—all new hires must complete this form within three days of starting work at Skidmore. Click here for I-9 Form Instructions.
New Employee Information Form:This form is completed by new employees and includes personal information that is required in Skidmore's database.
Summer Employee Information Form: This form is completed by summer employees and includes personal information that is required in Skidmore's database.
Temporary Assignment Forms
This form is used to allow exempt and non-exempt employees the opportunity to gain experience in other positions within the college while at the same time helping the college realize salary savings. It should be completed by both the employee and the employee’s immediate supervisor and returned to Human Resources.

Temporary Help Request Form (non-union, campus only): This form is an aid to assist supervisors when planning a request for temporary assistance due to a departmental vacancy.
